Beyond benefit and tax-related information, here are some resources you may need as a PERS retiree.

Moving

Moving — especially into or out of Oregon — can impact your PERS beyond just an address change. Read more about the impacts and what things you need to update with PERS on our Moving webpage.

Health insurance

The following are resources who can help you with health insurance and Medicare.

  • PERS Health Insurance Program (PHIP) — PHIP offers health insurance coverage for you, your spouse, and dependents who meet the eligibility requirements. Visit the PHIP website for more details or to update your enrollment information.
  • Oregon Senior Health Insurance Benefits Assistance (SHIBA) — SHIBA offers certified, local counselors who can help with Medicare-related questions. Learn more on Oregon SHIBA’s website.

Account access

For someone to review your account or act on your behalf in the event of severe illness or death, you will need to provide authorization to PERS using the following forms. These forms are only valid while you are alive; read the next section to find information about what happens in the event of your death.

  • Account information release — The Authorization to Release Account Information form will allow you to name other parties who can receive information about your PERS benefits, including account balances and benefit estimates.
  • Power of attorney — The Special Power of Attorney for PERS form will allow you to designate others who can make decisions about your PERS benefits on your behalf.

When you die

A family member, beneficiary, or caregiver must immediately notify PERS of your death. PERS will request a photocopy of your death certificate.

If you participate in PHIP or the Oregon Savings Growth Plan (OSGP), these entities also will need to be notified.
